Prime Ministers Challenge Fund

There are no current activities being run through the Prime Ministers Challenge Fund initiative.

Historical activity included an attempt to reduce pressure on the local Accident and Emergency and Out of Hours services, our federated group of five local GP practices (known as the Ashfield North Federated Group) worked together on a scheme to provide additional hours in their surgeries, being available to all the patients of all five practices, not just our own registered patients.    

The five participating practices (all local to Sutton-in-Ashfield) are :-

Woodlands Medical Practice (ourselves), Willowbrook Medical Practice (incorporating Ashfield Medical Centre), Brierley Park Medical Centre, Skegby Family Medical Centre  and Harwood Close Medical Practice.

A second project was particular to Woodlands Medical Practice only and was a pilot scheme for early home-visiting of urgently unwell patients or those who are at high-risk of hospital admission.  The overall objective of this scheme was to keep people healthier in their own homes, avoiding unnecessary hospital admissions, or when hospital admission is appropriate ensuring this is done in a timely manner to avoid any further deterioration and therefore help people recover and be discharged more quickly.   A similar service has now been implemented across the patch.
