How to Register register_now

You can register by completing a registration form which can be obtained from the practice.  When you register, we will ask you to complete a ‘New Patient Questionnaire’ and to bring your completed paperwork into the surgery.

You will be invited to make an appointment to see the practice nurse for a new patient health check.  This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address.  You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

Catchment Area

As a general rule, the A38 forms the boundary to the South and East of the practice.  Access to the surrounding villages is as follows: Kirkby-in-Ashfield – to the entrance to Ashfield School; Huthwaite – to Ashland Road; Skegby – to Mansfield Road/Buttery Lane.  However, those who feel they live close to the boundary should contact the practice where we will be happy to advise further.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
